Permanent supportive housing is an intervention that combines affordable housing assistance with voluntary support services to address the needs of chronically homeless people. The services are designed to build independent living and tenancy skills and connect people with community-based health care, treatment and employment services. In addition to ending a person’s homelessness and increasing their housing stability, permanent supportive housing has been shown to improve health and well-being.

The Homeless Prevention and Rapid Re-Housing Program will provide financial assistance and services to prevent individuals and families from becoming homeless and help those who are experiencing homelessness to be quickly re-housed and stabilized. The funds under this program are intended to target individuals and families who would be homeless without this assistance. Every household assisted by this program is provided case management to help them identify needs, establish a safe living situation, and to create a plan that connects them to the resouces needed to help them maintain housing. Only help with utility assistance if they are in jeopardy of being evicted.

Offers furnished, single-room occupancy apartments with a kitchen, bathroom and living area for chronically homeless single adults for as long as they qualify for the program. Supportive services and case management are also provided which may include weekly goal plans, budgeting, employment readiness, and housing assistance. *Services are also available for veterans and individuals receiving mental health counseling
