Teen Transitional Housing/Shelter | Sojourners Care Network

Assists runaway and homeless youth in Southeastern Ohio through a network of emergency shelters and short- and long-term housing programs. Each young person entering housing receives help with immediate needs followed by case management to help them move from volatile, unsafe living environments to more stable housing. There are no housing facilities in Fairfield Co. at this time but youth from the county are considered for the program.

Homeless Shelter For Young Adults | Harmony House

Offers emergency and transitional housing for young adults age 18 to 24 who are homeless and/or are facing homelessness. Provides a home setting with amenities and support services that is smoke-free, alcohol-free, and drug-free. Offers facilities with large common areas, a full kitchen, living and dining area, bathroom and laundry facilities, and personal hygiene supplies. Provides support services such as transitional planning for short- and long-term goals, assistance with obtaining documents such as birth certificates and social security cards, educational assistance, and personal development workshops.

Next Step - Transitional Housing - Shelter | Axess Family Services

Provides 10 homeless shelter/transitional housing beds for Geauga County residents only. Also provides case management services to young adults. Clients will be paired with a trained case manager who will help develop a personalized case plan to foster independence. Participants will learn skills such as financial planning, job readiness, study skills, time management, goal-setting, problem-solving and critical thinking. Clients may access case management services even if they are not using the housing portion of this program.

Transitional Housing For Women/Families | Catholic Charities Diocese Of Toledo

Provides six bedrooms for single women and women with children in transition from homelessness to permanent housing. Program includes chores, group living, and employment activities. Families participate in case management to assist them in addressing the root causes of their current homelessness, including Mental Health treatment, Substance use treatment, finding employment (if needed) and permanent housing. Subsidies for permanent housing and referrals to public housing may be available.
