Administers home and community-based supports for older adults and those with disabilities. Programs include but are not limited to: PASSPORT, Assisted living Medicaid waivers, long term care consultations, caregiver support services, regional long term care ombudsman, home repair, healthy lifestyles and wellness programs, aging and disability resource center, Medicare Part D enrollment assistance. Additional services may include: legal assistance, case management, transportation, personal care, housekeeping, adult day care, home delivered meals, emergency response systems, and other community resources.

Graceworks Enhanced Living program offers residential services and a day program for persons with developmental disabilities.

Larger activities can include but not limited to, COSI, the Columbus Zoo, Columbus Clippers, the Wilds, Theater productions, Dayton Airforce Museum & many more. Provides many free to low cost off site activities and all transportation.

MRSI's adult day support offers small group settings that teaches new skills and knowledge.

Provides in-home services for adults 60 and older as an alternative to nursing home care. A social worker or nurse will visit the client to assess needs and create an individualized plan. Services may include adult day services, home delivered meals, personal care, medical equipment, minor home repairs/modifications, and homemaker services such as house keeping.

Adult Day Services is a skill development and work experience program that empowers individuals to set and work toward personal goals, develop and maintain daily living skills and be involved in their community. Our adult day service program offers many diverse opportunities to explore the community through activities and visiting places of interest.
